How It Works:

When you place an order you will receive a detailed receipt of your order via email describing your order(please keep this receipt). To access the form simply click on the rebate center logo on the top right-hand side of the page or the words rebate center no. 13 on the left side of the page. Fill out the rebate form and Click submit and then print the form off and save to mail later.

Steps to redeem your rebate:

Step 1: print off your receipt that you received via email when you first placed your order.

Step 2: return to this website access the rebate center, fill out the form and print a copy off.
